Taking Charge
"My parents don't need all the services yet, but I do have concerns. The suggestions I got (via the Taking Charge visit) were great and (the Assessor) was so patient. My dad falls a lot, so even learning about half-rails for his bed and lifelines for both of them - well, I'm looking into that right away. "Taking Charge" Free Personal Needs Assessment Visit
is a starting point for making long-term care decisions. "Taking
Charge" offers a free in-home assessment, funded by the State
of Ohio and provided locally by a Council on AgingSM consultant.
During the "Taking Charge" visit the consultant, a specially
trained registered nurse or licensed social worker, assesses the
client's needs, provides information on available services, discusses
eligibility requirements, as well as financial resources required,
and helps to create a long-term care plan.

Planning for Future Needs Includes:
- Identifying the cost of long-term care services - An important consideration is whether institutional or home-based services are utilized.
- Evaluating current and future needs - Personal safety, caregivers' schedules, quality of care, and location of services are issues that should affect decision making.
- Discussing the long-term care plan with a Council on Aging consultant - If you begin your planning today, you will have time to investigate a variety of long-term care resources.
