Service Providers
REQUEST FOR PROPOSALS: Title III and Alzheimers Services (NEW!)  

A Service Provider is an established business that contracts with Council on Aging of Southwestern Ohio (COA) through various programs to perform needed services, at a COA approved rate, for our consumers.

Some of the programs administered by COA are PASSPORT (Pre-Admission Screening System Providing Options and Resources Today); the ESP (Elderly Services Program) in Butler, Clinton, Hamilton and Warren Counties; and Title III of the Older Americans Act, Senior & Community Services and Alzheimer State Funds.

Among the many services offered are Adult Day Services, Adult Day Transportation, Companion Services, Emergency Response System Services, Environmental Services, Home Delivered Meals, Home Medical Equipment & Supplies, Home Repair/Accessibility, Homemaker Services, Independent Living Assistance, Medical Transportation, Personal Care in an Adult Day Service Facility, Personal Care Services, Respite Care Services and Social Work/Counseling. Not all services are offered in all programs or counties.

This page offers some basic guidelines to Agencies wishing to learn about contracting with COA as a Service Provider. Also offered on this page are links to information useful to currently contracted Service Providers, such as ESP Program Guidelines, Conditions of Participation and Service Specifications for various programs, and even manuals to guide Service Providers through certain processes, such as billing.
