Becoming A Provider

Becoming a Provider for a COA-administered program such as PASSPORT, ESPSM, or Title III/Alzheimer State Funds begins with a request for an application.

Applications for the PASSPORT program are accepted at any time. To obtain an application to be certified as a PASSPORT Provider, contact Council on Aging of Southwestern Ohio at 513-721-1025.

ESP (offered in Butler, Clinton, Hamilton and/or Warren Counties) has an open application period every two years. A notice will be placed in major newspapers in each county as well as on the COA website: www.help4seniors.org.

Title III/Alzheimer State Funds also have an open application period every two years. A notice will be placed in major newspapers in each county, as well as on the COA website: www.help4seniors.org.

For all programs, once your completed application (including all additional required data) is received by our Quality Improvement team, your agency will be assigned a Quality Technical Advisor (QTA) who will assist you in the application process, as well as review your agency to determine whether your agency meets the criteria for certification as a Service Provider for that particular program (PASSPORT, ESP or Title III/Alzheimer State Funds) and the service(s) which you applied to provide. Once your agency is certified, your QTA will continue to monitor and provide assistance to your agency.

Certification for the PASSPORT program also requires approval by the Ohio Department of Aging (ODA) and the Ohio Department of Job and Family Services (ODJFS). Our Quality Improvement Team will forward their recommendation for your agency’s certification to ODA/ODJFS on your behalf.

ODA and ODJFS do not have to approve applicants for the ESP and Title III/Alzheimer State Funds programs.

After your agency has been certified to be a Provider, our Contracting Team will be notified by your QTA to send you an agreement (contract) for the program(s) for which you are certified.
