If you have a chronic health condition such as diabetes, high blood pressure or heart disease, you know the day-to-day work of managing your symptoms can be a grueling task. You may be tired all the time, confused about your medications or even worried about your out-of-pocket medical costs.
If this sounds familiar, the Healthy-U program may be for you.
There are two ways you can get involved in this free program: 1) participate in a local workshop with others just like you, or 2) learn how to put your own experience managing a chronic illness to use as a Healthy-U facilitator.
Participate in a local Healthy-U Workshop
Healthy-U is for adults of any age and their caregivers with any type of chronic health condition (diabetes, high blood pressure, heart disease, fibromyalgia, arthritis, sickle cell and others). It is an evidence-based program that focuses on problem solving and building self-confidence to help people maintain their health and manage chronic health conditions. Become a Healthy-U facilitator
If you`ve learned to manage a chronic illness and want to help others improve their quality of life, Healthy-U needs people like you!
The Healthy-U Chronic Disease Self-Management Program needs people like you to help others improve their quality of life and save on health care expenses. You`ll receive specialized training to help adults and/or their caregivers learn to manage the symptoms associated with a variety of chronic illnesses.
The free training will be held June 19-21, 8:30 am to 4:30 pm each day. The training will be held at Council on Aging`s offices, 175 Tri County Parkway, Springdale, OH 45246.
Once your training is complete, it is expected that you will: 1) lead two or more six-week courses in accordance with program guidelines and 2) maintain weekly attendance records and participant information to be turned in upon completion of the program.
