People enrolled in Transitions Carve Out Waiver moving to PASSPORT

Beginning this month, the Ohio Department of Medicaid is phasing out the Transitions Carve Out Waiver and the program will end on June 30. Statewide, more than 1,300 people will move from Transitions Carve Out to PASSPORT.
Transitions Carve Out was designed for people with disabilities who transition out of the Ohio Home Care Waiver when they turn 60. PASSPORT has now been expanded to offer the same services as Transition Carve Out. Individuals will keep their current service package and also have access to the full array of PASSPORT services, including those that are consumer-directed.
