Hospice of Cincinnati’sConversations of a Lifetime™ program is promoting National Healthcare Decisions Day (April 16) to inspire, educate and empower the public about the importance of advance care planning. An accident or serious illness can happen to anyone, at anytime. Talking to your loved ones now, before a medical crisis, is a gift you give to yourself and your loved ones.
Advance care planning involves making future healthcare decisions that include much more than deciding what care you would or would not want; it starts with expressing wishes, clarifying values, identifying health care preferences and selecting an agent to communicate healthcare decisions if you are unable to speak for yourself. There are resources to help you learn what you need to know and how to start the conversation. Download the Conversation Starter Kit to get started.
Once you have decided what you want and who will speak for you, write it down and share it with the people that will be responsible for making decisions for you. “Advance Directives” are legal documents (Living Will and Healthcare Power of Attorney) that allow you to plan and make your own end-of-life wishes known in the event that you are unable to communicate. Each state has its own rules and forms that you can download and complete yourself.
