The Ohio Department of Job and Family Services has launched 855-O-H-CHILD (855-642-4453), an automated telephone directory that will link callers directly to a child welfare or law enforcement office in their county.
Reports can be anonymous.
What information do I need to make a report?
- The name and address of the child you suspect is being abused or neglected
- The age of the child
- The name and address of the parents or caretakers
- The name of the person you suspect is abusing or neglecting the child and the address if available
- The reason you suspect the child is being abuse and neglected
- Any other information which may be helpful to the investigation
- You have the option of giving your name or reporting anonymously. Giving your name can help the investigator clarify information. The agency will not give your name to the person suspected of abusing the child.
Please Note: All of the above information is not needed to make a report. If you are not sure you have enough information to report, always err on the safety of the child. Children services screens all reports to determine if there is enough information to investigate.

Background
The AIDS Clinical Trials Information Service (ACTIS) was mandated by the Health Omnibus Programs Extension Act (HOPE) in 1988. This legislation required the U.S. Food and Drug Administration (FDA) to provide a telephone reference service to disseminate HIV/AIDS clinical trials information to health professionals and the general public. ACTIS, which was created by an interagency team at NIAID, FDA, NLM, and CDC, opened in 1989. During the 11 years of its existence, the service responded to more than 225,000 telephone requests for HIV/AIDS clinical trials information.
As new therapies and guidelines for the treatment of HIV/AIDS were approved, the need to provide up-to-date information in these areas became apparent. In 1994, six Public Health Service (PHS) agencies joined together to launch the HIV/AIDS Treatment Information Service (ATIS) as a vehicle to quickly convey this information to people living with HIV and health care providers. ATIS became the central dissemination point for the PHS treatment guidelines through the telephone reference service and the ATIS website. ATIS was supported by funding from many PHS agencies including NIH, the Health Care Financing Administration (HCFA, now the Centers for Medicare and Medicaid Services – CMS), CDC, and HRSA.
ACTIS and ATIS were merged into a single information service, AIDSinfo (https://aidsinfo.nih.gov), which became operational on December 1, 2002. infoSIDA (https://infosida.nih.gov), the Spanish-language companion website to AIDSinfo, was launched in April 2007.

Our application process requires that clients apply through registered Professional Service Providers who have a good working knowledge of the equipment they are recommending. Professional Service Providers are typically physical, occupational and speech therapists, but can also be physicians, nurses, case managers and government service workers.
Once an application has been submitted, the staff at Ability Found will search for the requested equipment after which any applicable fees will be proposed to the client for payment.
Once a project is approved, the equipment is picked up by the Professional Service Provider or their representative at the Ability Found warehouse. The Professional Service Provider will also fit the equipment to the client and train them as necessary in its use. When shipping is necessary for the approved equipment, it is sent to the Professional Service Provider for delivery to the client.

Access is a shared-ride public transportation service, providing curb-to-curb transportation for people whose disabilities prevent them from riding Metro buses. To use Access, you must first register and be approved for service, a process which can take up to three weeks.
About Access
The Americans with Disabilities Act of 1990 (ADA) was passed to remove the physical barriers that have kept persons with disabilities from fully participating in Metro society.
The ADA states that Metro`s regular bus service should be the primary means of public transportation for everyone, including people with disabilities. Under the ADA, Access serves as a "safety-net" for only those persons who do not have the functional capability to ride Metro buses. Disability alone does not qualify a person to ride Access under the ADA.
Customers made eligible for Access after January 1, 2004 are eligible for Access` ADA services only.
Access` service area and hours of operation are based on regular Metro service. Access service is limited to Hamilton County and within 3/4 of a mile of regular (non-express) Metro service and within the hours of regular (non-express) Metro service. For questions regarding Access policies call the Access information line at (513) 632-7590.
Rules of Riding
Access drivers are responsible for:
- Assisting customers on and off the vehicles lift or steps
- Securing your wheelchair or scooter
- Fastening every customer`s seat belt
- Contacting 911 in case of any medical emergency on the vehicle
- Collecting fares
Access drivers are NOT responsible for:
- Assisting customers in and out of buildings
- Carrying groceries and packages
- Installing child safety seats

Fare Deal
Metro`s Fare Deal program allows eligible riders to obtain a Fare Deal identification card valid for half-price fares on any Metro trip.
Eligible participants include:
- Adults 65 and older
- Medicare card holders
- Riders with disabilities
- Access card holders
Older adults are automatically eligible at age 65, proof of age is required. Medicare card holders of any age are also automatically eligible.
Riders with disabilities who don`t have Medicare and are not 65 must submit an application and be certified to qualify for the program. For Fare Deal information or an application call (513) 632-7540 (TDD Ohio Relay Service (800) 750-0750) Mondays, Wednesday, and Fridays, 8:15 a.m. to 4 p.m.
Fare Deal monthly stickers are available for $38.50. The sticker allows for unlimited rides each month. Stickers can be purchased at Metro`s sales office between the 15th of the preceding month until the 10th of the valid month.
Now Metro is more accessible than ever
With the addition of our new low-floor buses, the Metro fleet is nearly 100% accessible to people with disabilities. These new buses allow for easier wheelchair access and feature ramps allowing for faster and easier wheelchair boarding.
Metro standard bus fleet is also equipped with features to make riding easier for people with disabilities. Newer buses feature an automatic voice system to announce key stops, transfer points and other important information. Also, more than 90 percent of our buses are equipped with a wheelchair lift and two securement areas for wheelchairs.

Adult Protective Services (APS) works with Hamilton County’s elderly residents to help keep them safe.
APS operates (513) 421-LIFE (5433), Hamilton County’s 24-hour line for reporting elder abuse, neglect (by self or others) and exploitation of Hamilton County residents age 60 and older.
When calls are received by APS, caseworkers evaluate any risk to the safety and well-being of alleged victims. If they find that there is a risk, caseworkers work with families, social service agencies and other community resources to meet the person’s needs and to improve the situation.
APS caseworkers cannot force older adults to accept help. Mentally competent adults have the right to refuse APS assistance and intervention. APS will also not force competent older adults into nursing homes. When it is possible, APS will work with elderly adults to ensure they receive the help they need to stay in their own home.

The Role of the Bureau of Disability Determination
Once the application for disability benefits, or case is taken at the local office, it is forwarded to the Bureau of Disability Determination to assess whether the medical requirements are met for disability. Under the rules of Social Security, a person must be found unable to perform any type of work for a period of at least 12 months due to a physical, mental, or combination of impairments.
Under the Social Security Disability Program, an applicant cannot be found "25 percent" disabled as may be the case under the rules for worker`s compensation or Veteran`s benefits. The Social Security Administration uses a "Listing of Impairments" to guide disability assessment. This set of criteria is based on common types of medical conditions, such as arthritis, heart disease, and cancer. While every medical condition is not covered in the Listings, it does provide general guidance to assess them.
When the case is received by the Bureau of Disability Determination, it is assigned to a Disability Claims Adjudicator. This person reviews the application materials and may contact the applicant, or claimant, to clarify issues and obtain further information regarding treatment and limitations due to his or her impairments. The adjudicator then contacts relevant medical sources in order to obtain documentation regarding the impairments. At times, it is necessary to schedule consultative exams for claimants. This examination provides functional information to help determine what limitations may result from an individual`s impairment(s), and is not for purposes of providing treatment. Once sufficient medical evidence is obtained the adjudicator, using the Listings as a guide, will determine if the individual qualifies for disability.

BCEMA activities focus on the four primary phases of emergency management:

Shopping Shuttles
BCRTA provides area shopping shuttles at a reduced fare. These shuttles are available within the city limits of Hamilton, Fairfield, and Oxford only. Shuttles pick-up riders at a requested location similar to the Dial-A-Ride service and transport riders to a predetermined shopping area for $2 one-way. Pick-ups are scheduled around 9 AM and riders return between noon and 1 PM. Riders may visit more than one destination in the retail area if time allows. Shopping areas vary by day.
Community Connectors
BCRTA operates community connectors from Middletown to Hamilton and Oxford on a published schedule. Riders do not need to schedule these trips. Anyone may simply board the bus at a posted stop at the published time and pay the fare when boarding. Butler Metropolitan Housing Authority (BMHA)
Butler Metropolitan Housing Authority (BMHA) is a non profit (pseudo-governmental) entity established primarily for the purpose of developing and /or operating affordable housing according to rules established by the state and HUD. As a public housing authority BMHA is established under state law and is eligible to develop and operate public housing.
Public housing made available through BMHA provides affordable shelter for low income families either citizens or eligible immigrants. BMHA Public housing serves mostly very low-income tenants who pay no more than 30 percent of their adjusted income for rent.
BMHA also administers the tenant-based Section 8 program currently known as the Housing Choice Voucher Program. Section 8 vouchers helps very low income individuals and families rent safe and decent housing on the open market. This program pays the landlord the difference between what the individual/family can afford and the actual rent for the unit. In this program payment is made to the landlord on behalf of the qualified person and is not tied to a subsidized housing project. This form of assistance is portable and stays with the qualified person in the form of a Section 8 certificate or voucher.
BMHA Public Housing Program
The Butler Metropolitan Housing Authority owns and maintains apartments within the cities of Hamilton and Middletown. The units range in size from efficiencies to two bedrooms in their high rise communities and two to five bedrooms in their family communities. BMHA also has handicapped accessible units in varying sizes as well as single family homes scattered through out both cities.
The application process typically takes about 3 to 4 weeks.
The applicant is contacted when BMHA has a unit available to schedule an offering appointment, the applicant can accept or refuse the unit. If the applicant accepts the unit then an appointment is made to view the unit with a BMHA inspector. Then another appointment is made to come in and rent the unit, if the applicant in fact chooses to rent the unit.
Any missed appointment is automatically counted as a refusal, which means that the applicant is dropped to the bottom of the waiting list
Rent is calculated based upon the income of the house hold and cannot exceed 30% of the adjusted gross income. Utilities are provided in some units and utility allowances are provided elsewhere.
All applicants must wait 1 year before reapplying if they were evicted and/or abandoned their BMHA unit.
All applicants currently on parole or probation for any crime are denied housing.
BMHA enforces defined criminal related housing denials some which result in a 5 year wait, a 10 year wait or a denial for life.
BMHA receives funding from the US Department of Housing and Urban Development to maintain and administer the Public Housing Program.
BMHA Housing Choice Voucher Program
Funded by HUD, and formerly known as Section 8, the Housing Choice Voucher Program helps very low income individuals/families rent safe and decent housing on the open market. Individuals/Families find their own housing and pay a portion (not more than 40%) of their own adjust income toward the rent.
The Housing Choice Voucher Program assistance makes up the difference between the Individual /family's contribution and the actual cost of the unit.
Individuals/families can take full advantage of the wide range of housing available choosing the housing that best meets their needs. Individuals and families can take their subsidies with them should they move. This feature allows low income individuals/families the opportunity to move into healthy, safe and economically stable neighborhoods and to move closer to family and friends.

People who reside in Caracole House receive case management and referral services as well as mutual caring and 24- hour support from staff and residents. Caracole House is open to individuals with HIV/AIDS who have no appropriate alternative living option. The House is a four-unit apartment building that houses up to eleven individuals. Each resident has his or her own bedroom in a three-bedroom suite. Food, linen and household supplies are provided. An evening meal is prepared for community residents, and food is available for all other meals. Caracole House is supported by the United Way, the City of Cincinnati and other funders. Shelter Plus Care Shelter Plus Care is a rent subsidy program for homeless individuals living with HIV/AIDS and their families. HUD provides the rental subsidies. Caracole provides intensive case management and supportive services to help these individuals and their families find and maintain appropriate housing and stay together. Shelter Plus Care is partially funded by the US Department of Housing and Urban Development and the State of Ohio. Caracole Recovery Community The Caracole Recovery Community opened its doors in 2000 and offers housing and a program of recovery from chemical dependency for homeless adults living with HIV/AIDS. In addition to providing case management and referral services, the Recovery Community offers a 24-hour supportive environment allowing residents to attain self-sufficiency and to recover from addiction. Transportation CASS provides two kinds of vital transportation for those age 60 and older:
Guardianship Guardianship Assistance Program (GAP): The senior who is both very compromised and completely alone is a special, and often desperate case. Typically a group home or nursing home resident age 60 or older, this client may require a court-appointed Guardian to advocate for his or her interests, make health care decisions, and provide the comfort of human contact. Guardianship is a legally defined process. While it may pertain to the individual, the estate, or both, CASS's program is solely involved in the care and management of the person. GAP recruits and trains guardians for this compassionate task, providing legal and social services support. CASS also holds quarterly training sessions to monitor program quality and assure dignity in the face of decline. Guardianship services are available in Hamilton County only, although volunteers can live outside Hamilton County. Wilmington OH 45177 Description: Clinton Metropolitan Housing Authority (CHMA) is a non profit (pseudo-governmental) entity established primarily for the purpose of developing and / or operating affordable housing according to rules established by the state and HUD. CMHA as a public housing authority is established under state law and is eligible to develop and operate public housing. CMHA public housing provides affordable shelter for low income families either citizens or eligible immigrants. CMHA public housing serves mostly very low-income tenants who pay no more than 30 percent of their adjusted income for rent. Clinton Metropolitan Housing Authority: reports however that as a public entity CMHA currently does not own or maintain any public housing units .This Metropolitan Housing Authority reports it sold its properties to the Clinton County Community Action Agency several years ago. CMHA however does administer the tenant-based Section 8 program currently known as the Housing Choice Voucher Program. This section 8 voucher helps individuals and families by paying the landlord the difference between what the individual/family can afford and the actual rent for the unit. In this program payment is made to the qualified person and is not tied to a facility. This form of assistance is portable and stays with the qualified person in the form of a Section 8 certificate or voucher. The CMHA Housing Choice Voucher ProgramClinton Metropolitan Housing Authority administers the Housing Choice Voucher program in Clinton County. Clinton County receives an allocation of 293 Housing Choice Vouchers. This is a tenant-based Section 8 program. In this program the qualified household has the responsibility of finding their own housing to rent .Which may include single family homes, townhouses, apartments, duplexes and mobile homes. The program participant can choose any housing that meets the requirements of the program. Such housing is not limited to units located in subsidized housing projects. A housing subsidy is paid to the landlord directly by the public housing authority on behalf of the participating individual or family. The individual or family then pays the difference between the actual rent charged by the landlord and the amount subsidized by the program. This form of assistance is portable and stays with the qualified person in the form of a Section 8 certificate or voucher allowing the qualified person to move without loss of housing assistance. The CMHA Tenant Based Rental Assistance ProgramThe Tenant Based Rental Assistance Program (TBRA) provides rent and security deposit assistance. Recipients are enrolled in TBRA and then go on to be enrolled in the Housing Choice Voucher Program. The TBRA assisted approximately 20 to 25 individuals/families. The program is county funded and comes through the county commissioners office.
