
FastTrack Home
Easing the transition from hospital or nursing home to home.

FastTrack Home provides in-home care services when they are needed the most – as an older adult is leaving the hospital or nursing home. Services such as homemaking, home-delivered meals and medical transportation are set up before the client returns home and remain in place for up to 60 days to help in recovery and prevent unnecessary hospital readmissions. A care manager also works with the client to ensure they understand new medications, follow doctor’s orders and know what “red flags” to look for during recovery. FastTrack Home is paid for via the senior services tax levies in the counties where it operates and is offered at no cost to the client.
Eligibility for Services
- Butler County resident age 60+*
- Clinton County resident age 65+
- Hamilton County resident age 60+
- Warren County resident age 60+
- Discharging from a hospital or nursing home
- In need of help with everyday activities such as bathing, driving and preparing meals
- Able to be cared for safely in a home – not institutional – setting
- Agree to home visits from a care coordinator
Additional Information
If a client requests that services continue after 60 days in the FastTrack Home program, a full Elderly Services Program (ESP) eligibility assessment is conducted. Eligible older adults may be transferred to ESP for ongoing services, if they meet all requirements. Income is not used to determine eligibility for ESP. However, depending on financial resources, individuals may be required to help pay for the cost of their care through a co-payment (called cost-sharing).
