Adult day care option returns to Clinton County

Since 2020, Clinton County older adults and their caregivers have been going without an important service – adult day care. During the COVID-19 pandemic, adult day centers were closed as part of statewide lockdown orders and unfortunately, many never reopened.
But in November 2025, the Aging Up Community Center, located at 717 N. Nelson Ave. in Wilmington, began partnering with Extended Family Home Health Services to offer adult day care services to the Clinton County community. The center provides the space for Extended Family to operate the program, as well as meal service and transportation.
Robin Vaughn, MSN, owner of Extend Family Home Health Service, said she started the program based on her experience as a family caregiver and a desire to help people in the community who were struggling to help older loved ones remain independent.
Vaughn knows firsthand the challenges of being a working caregiver. She was working fulltime when she had to step in to help her father following a Parkinson’s diagnosis. Part of a large extended family with deep Clinton County roots, Vaugh said many family members in her generation were in the same situation. She wanted to find a solution that would help meet her family’s needs while allowing her to work and earn a living during the day.
So, in 2023, Vaughn started Extended Family Home Health as a way to meet her family’s needs, while also supporting other community caregivers and providing meaningful employment opportunities. The adult day service was added in 2025.
“I wanted to help people who were in the same situation I was in,” Vaughn said. “I wanted them to have better options and resources so they could continue working and taking care of themselves [as caregivers], while also keeping their family members at home. You can’t be a good 24/7 caregiver if you don’t take care of yourself. I love being able to offer these options.”
Through the partnership with the Aging Up Center, Extended Family Adult Day Services provides qualified staff, engaging activities, lunch and transportation – giving caregivers the peace of mind and respite they need, all while helping older adults remain independent in the community.
Vaugh, who has more than 20 years’ experience in home health care, said she and her team have built an adult day program that is personalized to participants’ needs and interests. For example, an intake survey helps staff understand what each participant likes to do – from crafts to farming to sports – and daily activities are created and planned around those interests.
Socialization is a big part of adult day services. Locating the adult day center within the Aging Up senior center provides more opportunities for participants to be around their peers, including through the congregate meal offered at the senior center.
Extended Family Adult Day Care is certified to provide adult day care services to eligible individuals enrolled in Clinton County ESP and PASSPORT, Ohio’s Medicaid in-home care program. Both programs are administered by Council on Aging in the county. A private pay option is also available.

Adult day services in Clinton County are federally funded with $19,364 (75%) available from the Administration for Community Living (ACL), U.S. Department of Health and Human Services (HHS) with a 25% match from local funding for FY 2026.
