Ohio Home Care Waiver
This in-home, long-term care Medicaid waiver program is for people under the age of 60 who have significant disabilities and/or mental health needs, including infants and children. COA provides care management services as a subcontractor for CareSource. We cover a multi-county region, which includes the Cincinnati and Dayton areas.
MyCare Ohio-Aetna and MyCare Ohio-Molina
MyCare Ohio is a managed care program for Ohioans who receive both Medicare and Medicaid benefits. Through the program, members have all their health and long-term care (including in-home care) coordinated through a state-approved health plan. Aetna Better Health of Ohio and Molina Healthcare of Ohio serve our region. COA provides care coordination for MyCare Ohio members via contracts with these two plans.
PASSPORT
PASSPORT is an Ohio Medicaid program that helps eligible older adults receive care and support in their homes, instead of a nursing home. It is Ohio’s in-home alternative to nursing home care. Without it, many disabled, low-income older adults would be forced prematurely into nursing homes. Council on Aging handles assessment, care planning and coordination functions for the program in southwestern Ohio.
Elderly Services Program
The Elderly Services Program (ESP) helps older adults remain safe and independent in their homes by providing in-home care services such as personal care, housekeeping, meals, transportation and more. ESP clients need in-home care but cannot access it through Medicaid or other programs/payers. ESP expands care already provided by family and friends and prevents unnecessary nursing home placement. COA administers the Elderly Services Program in Butler, Clinton, Hamilton and Warren counties. These programs are funded by county tax levies.
Positive Choices
An in-home care management program available to Humana Medicare Advantage members. Care managers help members learn to manage on-going health issues, set goals and make choies to positively impact health outcomes, and avoid future hospitalizations and emergency department visits. COA is contracted with Humana to serve at-risk members, including those with substance use disorders. Case managers conduct regular meetings with members, help members develop action plans to recognize triggers of their conditions, coach and provide tools to help members improve communication with their healthcare providers, and connect members to community resources to help them address health concerns and acheive goals.
