All-inclusive care to help Hamilton County older adults live their best lives at home

PACE of Cincinnati is for older adults who have disabilities, chronic health conditions or other health or long-term care needs, but want to remain independent in their homes and communities. PACE of Cincinnati is also for family caregivers who are struggling to juggle daily responsibilities, in addition to caring for their aging loved one.
To be eligible for PACE you must be:
- A Hamilton County resident
- Age 55 or older
- Eligible for nursing home care
- Able to live safely in the community with support from PACE of Cincinnati services
When you enroll, PACE of Cincinnati becomes the sole service provider for Medicare and Medicaid eligible enrollees, guaranteeing access to services, but not to a specific provider. The PACE care team provides and coordinates health care and other services and works hand-in-hand with you and your family to develop a personalized care plan.
As a PACE participant, you must receive all needed health care services, including primary care and specialist physician services (other than emergency services), from PACE of Cincinnati.
PACE of Cincinnati participants may be fully and personally liable for the costs of unauthorized or out-of-PACE program agreement services.

PACE of Cincinnati Services
PACE of Cincinnati provides services across all care settings, 24 hours a day, 7 days a week:
- At the PACE of Cincinnati day health center and via PACE’s network of local providers, participants receive primary care, therapy, meals, recreation, socialization, personal care and more.
- In the home, PACE offers skilled care, personal care supportive services, and supports such as ramps, grab bars, and other tools that help participants live safely at home.
- In the community, PACE offers access to local specialists and other providers.
- Transportation is provided to the PACE of Cincinnati day health center and to all PACE appointments, regardless of location.
- PACE is available 24/7. For PACE of Cincinnati participants and their caregivers, the PACE of Cincinnati care team is always only a phone call away, 24 hours a day, seven days a week.

PACE covers all Medicare Parts A, B and D benefits, all Medicaid-covered benefits, and any other services or supports that are medically necessary to maintain or improve the health status of PACE program participants.
