Active Day Adult Day Health Program is the largest provider of adult day services in the United States, serving seniors and adults with developmental disabilities. Active Day of Cincinnati has been providing services in the Cincinnati area since 1996. Open Monday thru Friday from 8am to 430pm, our mission is to provide a safe and secure environment where members can enjoy engagement in activities and companionship of new friends while giving much needed rest to caregivers. Using a dietician’s expertise, we provide nutritious meals including a morning snack, lunch and an afternoon snack. A licensed nurse is on staff to assist families and medical providers in keeping members healthy. Certified Nurse Aides assist with personal care as needed, including showers. Activity staff provide therapeutic games and exercise for health and enjoyment. Clavinova Connections- Recreational Music Making

Description:

The connection between music-making and wellness goes back to ancient times. In 2004 a group of scientists led by neurologist Dr. Barry Bittman, set out to determine whether recreational music-making activities could lower stress levels and stress-induced conditions such as heart disease and cancer. The research team discovered that participating in such activities not only lowered participants` stress levels and helped them relax, they also built confidence and self-esteem.  Recreational music making programs have been particularly effective among older adults in settings such as nursing homes, adult day centers and senior centers. 

Clavinova Connection is local example of a recreational music making program that are seniors may choose to get involved in.

Clavinova Connection
If you`ve always wanted to play piano, but don`t have any musical experience, Clavinova Connection maybe for you!  Clavinova Connection is a nationally-recognized program that uses digital pianos and a special curriculum to help help people with no prior experience successfully play their favorite musical selections, while at the same time reducing loneliness and stress, and boosting creativity.  Participants do not need to have any musical experience or knowledge to participate. Clavinova Connection is the first recreational music-making program offered to seniors in Ohio, as well as the first nationally to be used by a Council on Aging.

The Today Center for Adults is a program designed for people who need assistance, socialization, supervision, and care during the day.

 The purpose of the program is to help adults maintain the ability to live in their own home or a family member’s home and to offer support to families caring for older adults.

National Family Caregiver Support Program (Ohio Department of Agining)

Description:

National Family Caregiver Support Program

Through Title III of the Older Americans Act, the National Family Caregiver Support Program (NFCSP) provides direct help to eligible family and informal caregivers. In southwestern Ohio, Council on Aging combines resources from NFCSP with other local programs to provide assistance to as many caregivers as possible.

Types of assistance that may be provided through this program include:

  1. Information to caregivers about available services
  2. Assistance to caregivers in gaining access to services
  3. Counseling/Education/Training
  4. Respite care*
  5. Supplemental services — additional services and supports needed to help people stay independent in their homes (limited)*

*The care recipient must meet specific criteria.


Eligibility

  1. Family caregivers (of any age), of a person with Alzheimer`s disease or a related dementia may be served regardless of the age of the person with dementia
  2. Grandparents and other relative caregivers providing care to children (under age 18 years) may receive services at 55 years of age and older
  3. Grandparent or relative caregivers (age 55 and older) who provide care for adult children (ages 19–59) with a disability

Caring for a loved one can be hard work. However, the National Family Caregivers Association says that people who recognize themselves as caregivers are more proactive, engaged and confident. As a result, they provide better care and are able to do so longer, even as their loved one’s care needs increase. Caregivers who access and use support services also report fewer negative emotions, such as depression, anxiety and anger. By seeking and accepting help, along with planning for the future, you can provide the care your loved one deserves while also making sure your needs are taken care of.

Through the National Family Caregiver Support Program, your area agency on aging and other local providers are ready to assist you with supports that may include:

  • Training, resources, information and support groups;
  • Respite care, adult day and medical alert services;
  • Personal care, chore services and home delivered meals;
  • Yard work and home repairs;
  • Durable medical equipment and home modifications; and
  • More.

Northwest Adult Day Service is a Council on Aging contracted service provider offering the following services: adult day care adult day care transportation.


Ohio Home Care Waiver Program

Description:

Ohio Home Care is for eligible adults age 59 and younger (under 60).  Services include nursing, personal care assistance and/or skilled therapy, plus waiver-specific services such as home modifications, home-delivered meals, adult day health care, respite care, supplemental transportation, adaptive/assistive devices, and emergency response systems. Eligible participants must be financially eligible for Medicaid and be assessed for an intermediate or skilled level of care. Without the Ohio Home Care Waiver Program, participants would be at risk for hospital or nursing home placement. Click here for more information.  Approved participants instead receive care and services at home.  There may be a waiting list for this program.  Contact your county Department of Job and Family Services for information about applying for this program. 

  • Transitions MR/DD Waiver Program provides the identical services, providers and administration as the Ohio Home Care Waiver Program but serves consumers who were transferred from the Ohio Home Care Waiver because they were identified as having an ICF-MR (intermediate care for people with developmental disabilities) level of care. Contact your county Department of Job and Family Services for information about applying for this program.
  • PASSPORT provides the identical services, providers and administration as the Ohio Home Care Waiver Program but serves consumers who were transferred from the Ohio Home Care Waiver because they turned 60.
