Fast Track Home program named finalist for Health Collaborative award

Winners Announced November 1 at Inspire | Healthcare Event

Council on Aging's Fast Track Home program has been recognized as a finalist for The Health Collaborative’s 2017 Richard M. Smith Leadership in Quality Improvement Award. Each year, the nonprofit healthcare improvement organization hosts a dinner and awards event to recognize and celebrate the important work being done to advance the triple aim of healthier people, better care, and lower costs in Greater Cincinnati and Northern Kentucky.

Fast Track Home is an innovative program that improves the quality of care for hospitalized older adults by expanding the reach of the successful Elderly Services Program (ESP) in Hamilton and Clinton counties.  It connects older adults hospitalized at participating hospitals to essential in-home care services when they are most needed – at the time of discharge. 

With support immediately available at home, these patients – and payer sources – can avoid costly nursing home placement as well as hospital readmissions, which often occur when patients aren’t ready to care for themselves without help. Fast Track Home helps patients recover safely at home, where they are most comfortable and where they most want to be.
