A Geriatric Care Manager is a health care professional (usually a social worker or nurse) with aging-related expertise and familiarity with services available to assist with care of our older adults. They are especially useful for families who are assisting with care from a long distance. They usually start with a comprehensive assessment. Fees are charged to conduct the assessments, arrange services, and monitor services provided. Geriatric care managers can be hired on a one-time basis or to provide ongoing assistance.
