Aging and Disability Resource Connections (Call Center)

Council on Aging`s Department of Aging and Disability Resource Connections (Call Center) is the "front door" to our organization. This is where you get answers to your questions about aging, connect to community resources and begin the process of applying for our home and community-based care programs.
As the Area Agency on Aging for our region, Council on Aging is a central resource for unbiased, trustworthy information that can help you navigate the complex world of services for older adults, caregivers and people with disabilities.
In addition to answering questions, Call Center staff members may begin the screening process to determine your eligibility for Council on Aging programs, such as PASSPORT and the Elderly Services Program.
An assessor may then come to your home to complete the eligibility process or simply consult with you and family members about long-term care planning, programs and support services. In-home assessments are for people of all ages who are looking for information and answers on a variety of aging-related topics
The Call Center includes a coordinator for the Ohio Senior Health Insurance Information Program (OSHIIP) who is available to: help seniors make decisions about Medicare coverage; facilitate appeals for denied coverage; and act as a third-party mediator between patients and providers when coverage problems arise.
To inquire about enrolling in home care, community-based or assisted living services; pre-admission review for nursing homes or to schedule a free in-home consultation: 513-721-1025 or toll free: 800-252-0155. Press 1 to be connected to our Call Center. You can also send us a message through our website.
