Council on Aging honored for improving transitions of care for hospitalized older adults

Council on Aging’s innovative program for helping hospitalized seniors connect with essential in-home services received The Health Collaborative’s Richard M. Smith MD Leadership in Quality Improvement Award. The award was presented to Council on Aging’s Fast Track Home program at the organization’s annual Inspire|Healthcare Awards Ceremony on November 1. The award recognizes an individual or team who exemplifies commitment to quality improvement, makes it an organizational priority, and serves as a role model for those working to enhance the quality of care within their organization and throughout the community.

Fast Track Home does just that. It improves the quality of care for hospitalized older adults by connecting them with in-home care services when they are most needed – at the time of hospital discharge.

“Fast Track Home is good for patients and our region’s healthcare system,” said Council on Aging President and CEO, Suzanne Burke. “It helps seniors recuperate in the most comfortable setting – their own home – and it prevents costly nursing home placements and hospital readmissions.”
