In-home and Community Care

In-home care is a simple phrase that encompasses a wide range of health and social services. These services are delivered at home to people who are recovering, aged, disabled or chronically or terminally ill. Services include medical, nursing, social, or therapeutic treatment, and/or help with essential activities of daily living. Programs help pay for in-home services for those who are eligible. These programs help older adults stay safe and independent in their own home. They supplement care already provided by a family caregiver.

Who can benefit from home care?

A person who:

  • can be discharged from a hospital or nursing facility, but needs additional care at home.
  • needs short-term help at home because of outpatient surgery.
  • needs additional help to live independently due to illness, disability, or aging.
  • has a chronic condition such as heart disease or diabetes.
  • is terminally ill and wants to spend his or her remaining time in the dignity and comfort of home.

Generally, home care is appropriate whenever a person prefers to stay home, but needs ongoing care that cannot be provided by family and friends alone. Most people prefer this option over institutional care, if possible.

Activities of Daily Living

What are ADLs and IADLs?

Activities of Daily Living. These include: bathing, dressing, grooming, walking, getting in/out of bed, chair or tub, eating, and using the toilet.

IADLs are Instrumental Activities of Daily Living. They include: shopping, meal preparation, house cleaning, heavy chores, laundry, telephoning, transportation, handling legal or financial matters, and managing medication.

To determine whether a person is eligible for home care services, agencies such as Council on Aging measure a person's ability to perform ADLs and IADLs. To enroll in COA home care programs, a person must be impaired in (need help with) ADLs and IADLs.

Types of home care services

Services fall into two categories: 1) skilled and 2) home support or custodial care.

Skilled Services
Skilled services are a higher level of care. They are usually provided by trained medical professionals. These services include: skilled nursing services, physical, occupational, and speech therapy

Home Support/Custodial Intermediate Care Services
Custodial care does not require specialized training or services. Custodial care may include any of the following:

  • Companion Services - provides socialization
  • Chore or Environmental Services - pest control, waste removal and heavy cleaning
  • Emergency Response Systems - 24 hour a day protection for emergency response and medication administration
  • Home Medical Equipment and Supplies
  • Home-delivered Meals
  • Home Modification and Repair - safety-related repairs and modifications, such as grab bars, ramps or dead bolt locks
  • Homemaker Services - help with meal preparation, shopping, light housekeeping, and laundry
  • Independent Living Assistance - assistance with finances, personal business and telephone support
  • Nutrition Counseling - diet and nutrition instruction
  • Personal Care Services - help with bathing, dressing, and grooming
  • Respite for Caregivers - time off for the primary caregiver.
  • Social Work Counseling - dealing with issues and problems

Who provides home care?

Health Agencies (HHA): People who need skilled home care services usually receive their care from a Home Health Agency. The term Home Health Agency often indicates that a home care provider is certified by Medicare, but this is not always the case. A Medicare-certified agency has met federal minimum requirements for patient care and management and can provide Medicare and Medicaid home health services.

Homemaker and Home Care Aide Agency: These agencies employ homemakers or home care aides and companions who provide meal preparation, bathing, dressing, and housekeeping. Some states require these agencies to be licensed and meet minimum standards established by the state. Ohio has no licensure law for these agencies. Some agencies receive certification (not licensure) from the Ohio Department of Health. These certified agencies can bill Medicare and Medicaid for the care they provide.

All home care providers that have contracts with Council on Aging (COA) must comply with Conditions of Participation in order to provide service for COA clients. COA providers are also surveyed once a year to determine how well they comply with these quality standards.

Home Medical Equipment Company: These companies provide home care clients with products such as wheelchairs, walkers, catheters, and wound care supplies. Home medical equipment and supply companies are required to meet federal minimum standards if they are paid through Medicare.

Hospice: Hospice care involves a team of skilled professionals and volunteers who provide comprehensive medical, psychological, and spiritual care for terminally ill patients and their families. Hospice care also provides related medications, medical supplies, and equipment. It is based primarily in the home, so it helps families stay together. Most hospices are Medicare-certified and licensed according to state requirements. Payment for hospice care is available as a benefit under Medicare, Medicaid and most private insurance companies. Pharmaceutical and Infusion Therapy Companies: These companies specialize in the delivery of drugs, equipment, and professional services for people who need intravenous or tube feedings. These companies employ pharmacists who prepare solutions and arrange for delivery to patients. Some pharmaceutical and infusion therapy companies are home health agencies, certified by Medicare. In addition, some states require these organizations to be licensed.

Resolving a home care problem

If you invest some time and find out about home care agencies before beginning services you will most likely receive high-quality home care that is safe and effective. If a problem develops or you would like to file a complaint, notify the home care agency's manager. You may also contact the regional Long-term Care Ombudsman.

Description:

Emergency Response Systems -- commonly referred to as LifeLines -- are bracelets or necklaces that can be worn and activated if the user falls or becomes incapacitated and cannot reach the phone to call for help.  Emergency Response Systems are monitored 24 hours a day, 7 days a week and operators are trained to respond appropriately if the system is activated by the user.  Many people who receive in-home care services through Council on Aging receive an Emergency Response System as part of their care plan.  The service can also be subscribed to and paid for privately.  Contact Council on Aging for a list of local providers: (513) 721-1025.

Description:

For many people, getting at least one nutritious meal a day can be a challenge. Physical or cognitive disability, illness, and lack of support or transportation are common barriers that prevent many people from preparing food or traveling to a group meal location such as a church or senior center.  

To offer access to nutritious meals, Council on Aging (COA) provides home-delivered meals through 1) PASSPORT (Ohio's Medicaid waiver home care program), 2) locally-funded Elderly Services Programs, and 3) the Ohio Home Care Waiver program (Ohio Medicaid waiver program for people under age 60). Each year, COA provides more than 2 million meals to approximately 10,000 homebound older adults in Butler, Clermont, Clinton, Hamilton and Warren counties.

Eligibility

Eligibility for home-delivered meals is determined by nutritional need and criteria are derived from funding sources. To qualify for home-delivered meals, you must meet all of the following criteria:

  • age 60 or older (except for the Ohio Home Care Waiver where there is no age criterion)
  • unable to prepare your own meals
  • unable to participate in a congregate nutrition program because of physical or emotional difficulties
  • lack another meal support service in the home or community

Home-delivered meals may also be provided to the spouse of an eligible person, who lives in the home of the eligible person, regardless of the spouse's age or abilities.  This is determined on a case-by-case basis. 

Meals are prepared by one of our quality meal providers and delivered directly to your door by a friendly volunteer or staff person. Delivered meals can be hot, chilled or frozen and can be prepared to meet special dietary needs (diabetic, dietary restrictions, etc.).


Cost

Home-delivered meals through Council on Aging are free of charge. However, our funding does not cover the full cost of preparing and delivering meals. Donations allow us to reach out to more people and help with the costs for preparation and delivery. Donations of any size are accepted and appreciated.  Your meal provider will explain how you may make a donation.

Golden Buckeye Card, Ohio Long-term Care Consumer Guide, and Ombudsman

Ombudsman

Ohio's Office of the State Long-term Care Ombudsman advocates for people receiving home care, assisted living and nursing home care. Paid and volunteer staff work to resolve complaints about services, help people select a provider and offer information about benefits and consumer rights.

Ombudsmen do not policy nursing homes and home health agencies. Instead, they work with providers, residents, their families and other representatives to resolve problems and concerns. Ombudsmen advocate a person-centered approach to meeting the needs and honoring the preferences of their clients. Ombudsmen link residents with services or agencies, offer advice on selecting long-term care providers, inform consumers about their rights and provide information and assistance with benefits and insurance.


Ohio Long-term Care Consumer Guide

 

The Long-Term Care Consumer Guide provides information about nursing homes and assisted living facilities to help consumers, family members and professionals search for the appropriate facility to meet an individual's needs. If you have determined that you need to move to a nursing or assisted living facility, you can start to compare facilities by using the Ohio Long-Term Care Consumer Guide.

The Golden Buckeye Card

Golden Buckeye is a discount card available for residents of Ohio, age 60 years and older and adults 18-59 with permanent and total disabilities. The program offers discounts on meals, entertainment, merchandise, and services at participating businesses throughout the state. Look for businesses that display a "Golden Buckeye Honored Here" window decal.  There are nearly 20,000 participating merchants statewide. 

Seniors who have an Ohio driver`s license, an Ohio ID card or are registered to vote will automatically be sent a card in the mail when they reach age 60. The packet will have information on how to use the Golden Buckeye Card. If you do not receive the Golden Buckeye Card when you become eligible, you can pick up a Golden Buckeye application at a local senior center or your local branch library.

The Society of St. Vincent de Paul in Cincinnati is a non-profit organization providing a unique style of personal assistance with food, clothing, medicine, rent, utilities, transportation and companionship to people facing economic, emotional or spiritual crises.

The Charitable Pharmacy

The Charitable Pharmacy is currently open on Mondays, Wednesdays and Thursdays each week to certify up to 12 new clients each day for up to six months of prescription coverage. To get enrolled you will need proof of residence, verification of income and expenses for all household members, and original or faxed prescriptions. The pharmacy serves clients from Butler, Clermont, Hamilton, and Warren Counties.

Edyth & Carl Lindner Choice Food Pantry

For those requesting assistance with food, the Edyth & Carl Lindner Choice Food Pantry is open every Tuesday, Friday and Saturday 10 a.m. to noon at our West End Office at 1125 Bank Street.  Those living in the following zip codes are welcome to come: 45202, 45203, 45214, 45219, 45225, 45229, 45232. For those living outside the zip code range, please call (513) 421-0602 to locate a food pantry in your area. 

Please bring the following documentation with you:

  1. Picture ID, Social Security Cards or a printout for the entire household.
  2. Proof of address, for example a recent utility bill or any piece of mail with your name and address on it, postmarked within the last 30 days.

Homelessness Prevention

St. Vincent de Paul`s help is diverse and far-reaching, but it begins in prevention.  For those living paycheck to paycheck an unexpected expense such as a car repair or a child`s medical bill can set the whole household over budget.  Often a small investment to help with a rent or utility bill will save a family from eviction or having water or electricity turned off and ultimately catches them before they become homeless.

TriHealth Driver Rehabilitation Program
For disabled drivers of all ages, TriHealth offers an objective evaluation of the skills necessary for driving. You must have a physician prescription to participate in this test. TriHealth can perform the in-clinic evaluation at Good Samaritan Hospital or the TriHealth Fitness & Health Pavilion.

Parish Nurses
In 1998, Good Samaritan Hospital Foundation, with a grant from what is today called the Health Foundation of Greater Cincinnati, hired two nurses to cover eight schools. Their work in the schools complements that of 10 parish nurses based at churches in Over-the-Rhine, North Fairmount, Bond Hill, Winton Terrace/Winton Hills, Clifton/Mt. Auburn and Walnut Hills.

Bethesda Alcohol and Drug Treatment Program.
The highly-regarded program, established in 1978, is the longest-running of its kind in Greater Cincinnati. It provides a path to recovery for those at risk of losing their families, careers and health to chemical dependency.

The Bethesda Alcohol and Drug Treatment Program continues to incorporate treatment advances into an already well-established program, which uses traditional tried-and-true approaches including family involvement, extended continuing care and self-help group support.
