COA welcomes clients from PACE program (Update)

This September, Council on Aging will welcome approximately 350 new clients as our region’s PACE program comes to a close.

PACE, the Program of All-Inclusive Care for the Elderly, is a Medicare and Medicaid program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility. TriHealth decided not to continue its contract with the Ohio Department of Aging and recently announced the closing of the PACE program, called SeniorLink. TriHealth’s program officially ended August 31, 2014. A separate PACE program continues to operate in the Cleveland region.

Around 400 adults ages 55 and over are affected by the closing, with nearly 350 opting to transition into COA programs. The majority will be enrolled in PASSPORT, with others enrolling in the Assisted Living Waiver and the Ohio Home Care Waiver.

According to the manager of COA’s Projects Department, Jeanne Wallman, COA staff have been working since late June to complete in-home assessments for every PACE participant who wishes to transition to COA programs. The assessments were prioritized based on need and were on track to be completed by an August 22 deadline.  Wallman said this deadline will allow time to have service plans in place at the beginning of September in order to provide a seamless transition and continued care to the transitioned clients. 

(Update) As of August 28, 2014, Council on Aging staff had completed all scheduled PACE visits and transitions.  This work included 327 in-person visits and establishing services for 317 consumers. COA successfully transitioned from PACE:

  • 25 consumers to the Assisted Living Waiver
  • 34 consumers to the Ohio Home Care Waiver
  • 258 consumers to PASSPORT

"This transistion impacted everyone from COA's front door to the care management staff and everyone in between," Wallman said. "On our final conference call [regarding the transition], both Ohio Department of Aging and TriHealth SeniorLink staff complimented our staff on doing a great job during this transition."
