New online referral form, updated reference guide make it easier to connect to COA services

Council on Aging is making it easier to connect to programs and services that help eligible older adults and people with disabilities live safely and independently in their homes and communities.

In March, COA launched an online referral form that helps professionals (healthcare workers, first responders, social workers, service providers, etc.) quickly and securely refer potentially-eligible patients, consumers, and clients for services such as home-delivered meals, personal care, care management, transportation, an emergency response device, skilled nursing, and more. Previously, referrals were made over the phone or via fax.

“We want to make it as easy as possible for professionals to connect with us,” said Lindsay Alexander, COA’s Aging and Disability Resource Center Business Manager. “In today’s fast-paced health care environment, we need a tool that lets people make referrals when it’s convenient for them – not just when our call center is open.”

Online referrals are accepted 24 hours a day, seven days a week. Aging and Disability Resources staff respond to referrals in 1-2 business days (response times are the same if referrals are made by phone or fax). People who make online referrals also have the option to be notified of the referral outcome.

COA also has a Contact Us form which can be used for general questions about our programs and services or other community resources (not referrals).

Additionally, COA updated the popular Reference Guide to Home and Community-based Care. The Reference Guide is a handy, one-page document that includes information and eligibility criteria for the breadth of home and community-based long-term care services and resources in our area, including the Elderly Services Programs, PASSPORT, MyCare Ohio, Care Transitions, and Hospice. It is available in English, Spanish and Russian. The fact sheets may be printed from the website, or request copies by sending a message to newsletter@help4seniors.org.  Spanish and Russian versions must be printed from the website.
