COA to continue care management services for Ohio Home Care Waiver

Effective July 1, Council on Aging was awarded a contract with the Ohio Department of Medicaid to provide care management for people enrolled in the Ohio Home Care Waiver. Council on Aging has been providing care management services for Ohio Home Care Waiver clients in a 21-county region since January 2014.

The program is an in-home long-term care option for eligible children and adults who have significant disabilities and/or mental health needs. It provides cost-effective home and community-based services to help people remain in a community setting, rather than a nursing home.

The program is administered by the Ohio Department of Medicaid through the Bureau of Long-term Care Services and Supports. It preserves independence by giving participants greater control and choice over their care, including where they receive care, the types of services they receive and who provides their services.

Services include help with personal care (bathing, dressing, and grooming); respite for caregivers; adult day service; emergency response system; home-delivered meals; nursing; home modification; skilled therapies; supplemental adaptive/assistive devices; and supplemental transportation.

Via a competitive process, Council on Aging applied for and received a contract to provide care management for the waiver program in a 21-county region. Area Agency on Aging 3 (Lima area) is contracting with Council on Aging to provide care management in their counties. Council on Aging is also working with other individuals and organizations who bring specialized services and expertise.
